We propose to run an exchange programme for UK general practitioners (family doctors) to travel to Europe and observe clinical practice under the supervision of an experienced host. The project will target two specific groups: doctors who are undergoing vocational training in family medicine, and those who have qualified to work as family doctors within the last year.
This project, known as the Hippokrates exchange programme, is now well established within Europe. The programme aims to encourage exchange and mobility among junior doctors in the course of their professional formation as family doctors, thus providing a broader perspective to the concepts of family medicine at both professional and personal levels. Hippokrates was originally started 10 years ago when it was recognised that junior doctors would benefit from the opportunity to directly experience delivery of family medicine in different European countries. This was seen as a vital addition to training, making family doctors better prepared to deal with life in an increasingly multicultural society and workplace. The UK project first received funding in 2010 and this enabled us to develop and expand the Hippokrates programme, providing new learning experiences for our participants, as well as forging stronger links with our counterparts across Europe.
Placements take place in a number of different European countries, where there are already established links with partner organisations. Each placement will be 2 weeks long. The project will take place over a 24 month period. Placements will be staggered according to when the participants can take study leave from their jobs within the National Health Service (NHS).
The participants will be chosen from across the UK and will be representative of the diversity within the junior doctor population in terms of age, sex, religious beliefs and ethnic origin.
In preparation for the visit, the participant will be expected to draw up a list of learning objectives following discussion with their host and with reference to the WONCA GP curriculum. These objectives also relate to UK requirements for training and continuous professional development (CPD). Guidance regarding language training will also be provided.
When overseas, the programme consists of clinic sessions, shadowing various members of the medical team as well as attending local training sessions. The participant will have protected tutorial time with their training supervisor, to allow evaluation of progress. They may live with a local doctor, allowing time for informal discussion about their experiences. On returning, the participant and the host sign a document that shows the educational timetable was adhered to. Learning outcomes are submitted to the JIC and the European host, and are compared with objectives. A written summary is completed where the participant can reflect on the quality of the placement.
The main outcomes will be learning in the core areas relevant to general practice as outlined by WONCA. As such, doctors will also gain knowledge that will contribute to their training in a number of key skills and competencies outlined in the RCGP curriculum, linking in directly with work towards passing the MRCGP examination which is the licensing exam for GPs in the UK. For newly qualified family doctors, the outcomes link in directly with requirements for CPD and revalidation. This will ultimately help the participants to maintain their UK licence to practice and increase their attractiveness to employers. Mobility projects also help to develop individual general practices and the NHS as a whole.
Our last funded project in 2012 was very popular and was oversubscribed. This year we wish to apply to expand our project by offering places for 51 participants, from all educational faculties in the UK. Having now successfully coordinated two previous projects, we feel we have the resources and the experience to offer more applicants this invaluable experience. We have also worked to develop partnerships with a wider variety of countries, thus offering greater choice and broader experience.
