Case Management in employment promotion an health care

In the field of tension between rising costs and increasing complexity the concept of Case Management (CM) has been established in Germany and the UK. CM regards the private life of a person in the context of the available systemic options. This requires both technical and methodological skills, competences which exist in the EU's partner countries Latvia, Poland, Greece and Slovenia only selectively. Taking into consideration the labor- and economic policy it is a challenge in all partner countries to adapt new and innovative best practices and tools and to use them actively. The qualification of Case Management experts in Greece, Latvia, Poland and Slovenia, using the successful experiences of vocational training from UK and Germany was a key success factor and primary goal of this project. The "Case Management" educational program, which is tested and DGCC certified, was transferred to the partner countries. In the course of project requirement profiles for "case managers" in the area "employment" and "health" were determined and considered while creating the educational program for partner countries. The thematic focus in the countries of Slovenia and Poland was on the "employment" and in Greece and Latvia on the "health care". Teaching staff was qualified and took over the testing of the training program in the participating countries.
